In RA, the synovium, the tissue that lines the joint, mistakenly becomes a target for the immune system. The immune system cells release inflammation-causing chemicals which cause inflammation in the synovium. The synovium creates a fluid called synovial fluid, which lubricates the joint allowing for smooth movement. When the synovium becomes inflamed and damaged, it fails to produce this lubricant and the joint does not move as freely. If the inflammation of the synovium continues over time, the cartilage and eventually the bone become damaged. The joints can then become deformed and misshapen, and movement becomes limited.

RA affects more than just the hands and other smaller joints; it can affect the entire body making it a systemic condition.

Symptoms

  • pain in multiple small joints for six weeks or longer,
  • morning joint stiffness lasting longer than 30 minutes,
  • bilateral pain,
  • loss of energy and appetite,
  • low grade fever,
  • dry eyes and mouth (Siogren’s syndrome),
  • and rheumatoid nodules, or lumps, which can grow beneath the skin.

Trouble breathing, anemia and inflammation of the blood vessels can also be symptoms of RA.

Joints affected by RA may be tender, warm and swollen, typically in a symmetrical pattern – meaning if one side is affected the other side usually is affected as well. RA shows up in the smaller joints first, so wrist and finger joints are affected most often, but other joints such as the neck, shoulders, elbows, hips, knees and ankles along with the feet can be affected. While there is a great deal of variation in symptoms, once someone shows signs of RA, those symptoms usually last for years.

Drug treatment for RA involves drugs called disease modifying antirheumatic drugs (DMARDs) that relieve symptoms and slow the progression of the disease. Analgesics like non-steroid anti-inflammatory drugs and NSAIDs may also be used to reduce swelling, pain and fever.

Massage and Rheumatoid Arthritis

While massage can aid in increasing mobility and alleviating muscle tightness resulting from chronic pain, it cannot do much to alleviate pain from deformed or degenerating joints. Moderate pressure Swedish massage and myofascial release are the two types of massage that research has proven offer benefits for those with RA, but there may be other techniques that reduce pain and increase mobility as well.

Massage therapy has been shown to reduce pain and increase joint mobility as well as decrease depression and anxiety in clients with RA. In one study, those with RA who received moderate pressure massages enjoyed less pain and greater mobility and grip strength than those who received lighter pressure massage therapy (Field, et.al., 2013). Another study showed that the stress hormone cortisol decreased immediately when RA patients received massage therapy (Field, et.al., 1997).

A 2011 study showed that myofascial massage, applied three times per week for two weeks, provided significant pain relief from RA symptoms. Myofascial release works through the application of sustained moderate to deep pressure, which allows the muscle to lengthen, reducing strain on the joints from muscle tension (Cubick, et.al., 2011).

Deep tissue massage techniques or trigger point therapy on clients with RA, should be limited, as these techniques might trigger a flare up of inflammation, especially near joints affected by the disease. When employing these techniques, it is important to work slowly, giving the client time to react, and to not overdo it. Let the client “live with” the work after a short, limited amount of deep work to see how their body reacts. If they are okay and do not have an increase in inflammation in RA-affected joints after the session, you can do a little more deep work at the next appointment. If they have a flare up of symptoms, avoid deep tissue techniques. Communication from the client is key. Massage should never be more painful than a good workout. Discomfort is one thing, but especially when dealing with RA, pain is not optimal.

Contraindications

Basic contraindications for massage therapy apply to those clients with RA as well; avoid massage when there are open wounds, fever, skin rashes or irritation, or deep vein thrombosis. Clients with any significant health concerns should be cleared by their physician before getting massage therapy.

When working on clients with RA, joints that are in an acute stage of inflammation and are warm, red, or extremely painful should be avoided. The therapist should check in with the client regularly to ensure that the pressure is not too deep, as it is important to ensure the massage does not trigger muscle contraction and cause more tension and pain. If a client is having a significant flare up of symptoms, massage should not be performed until symptoms calm down. Joint mobilization and stretching should be performed with care, as joints may be damaged or compromised.

Massage therapy has been shown to significantly reduce pain, increase mobility and reduce stress and anxiety for clients with Rheumatoid Arthritis. Massage therapy should be light to moderate, and care should be taken with inflamed and potentially damaged joints. Clients with RA live with pain as a constant companion and truly appreciate any intervention which offers relief from that pain.

Communication with the therapist as to medications taken is important. Depending on the medication considerations will be made pertaining to how the medications affect the body including pain relief, muscle relaxation, nerve reaction, dehydration, time of last dose etc.

A couple of days ago, I awoke in a panic with this on my mind and have been simmering on it trying to figure out a way to share this very important information with my clients and colleagues.

How many times have my clients forgotten to tell me what medicines they are taking? OMG a million times over! And this is so very important not only for their health but for mine! Some medicines are contraindicated for massage because while taking it, harm can come to the individual who is on those drugs. Some medicines are contraindicated because while taking it, the massage can be negated rather quickly. Some medicines are contraindicated because they are topical and can cross over to the therapist. Yes, I said that!

I have on my intake form What medicines are you taking for a reason. I have to be able to determine how and where I can work on you. I don’t just rub bodies.

Additionally, I have on my intake the direct question of Are you taking any topical medicines? This is doubly important!

I’m not sure why, but many people feel that if it is a cream or gel, it isn’t really a medicine. That is so not true! Just because it fades into your skin almost immediately does not mean it is not still working, or able to be transmitted to someone who touches that area. ESPECIALLY if that person is doing a bit more than just rubbing it and is actually reactivating the agent using oils and creams and friction!

A few years ago a colleague was having medical issues. She found out that she was overdosing on female hormones because a client she was seeing multiple times a week was using a topical version of the same thing she was taking. When my colleague found out she was ODing somehow I asked her if any of her clients were on hormones and taking them topically. We surveyed and got our answer.

I immediately changed my intake forms to include that.

Fast forward.

How many of you know a man with low testosterone? How much more prevalent is the advertisement of drugs for low T? How many people believe that talking about anything in that region is taboo and should be avoided at all costs? How many people are embarrassed about why they might have to take any kind of drug? If they are hiding it from their spouse or significant other, do you think maybe they might “forget” to put that on their intake massage when they are “just going for a bit of relaxation in a massage?” Absolutely!

Testosterone gel is rubbed onto the body (the insert I saw suggested the pectoral muscles, shoulders or belly) daily. How many male shoulders and pecs do you think I work on? Um every client male or female that comes in to see me gets worked on there! Female hormones are generally rubbed onto the hips or belly, and I work on just about every hip that comes into my office too .. male or female. Bellies I do work on, but not as often.

It clearly says on the package that females are NOT to touch treated areas. In fact some say women are not to touch the treated areas for at least 6 hours. And if so to wash hands thoroughly with alcohol. Others say it is never safe for a female to touch  a treated area and that a shirt should be worn during intercourse. Massage isn’t intercourse, but I’m doing quite a bit of touching.

I would say that massage is much more intense than mere touch.

OK … that’s just the hormones….

I’ve heard stories of clients coming in with pain patches who thought they were not working only to have the therapist have to stop the session because her tongue went numb from all the pain meds on the person’s body. That’s from patches not even creams! Who would have thought? Needless to say, because of this, I will not work on anyone who has a pain patch. Sorry, I don’t need a dose of pain meds. I need to be able to feel what I am doing to work on my clients.

OK … so what about other topicals?

Many people take topical steroids for different issues. Those are also able to be transferred to others. I don’t need steroids either, but thanks!

How many people out there forget that supplements are drugs? How many people forget that topical are drugs? How many people out there think that massage therapist just don’t need to know the medicines they are taking and don’t fill out the intake completely?

Please, I’m begging you. Be honest with your therapists! It’s not just about being able to take care of you the way we need to. It is about being able to take care of all of our clients and also ourselves. Please fill out your intake forms completely. Please be honest. We aren’t asking just to be nosy.

If you are coming in to get a massage and you are taking topicals, IF IT IS POSSIBLE … please apply in an area that I will not be working on. If your low back is hurting, I’ll need to work on your hips. If your neck and upper back are hurting, I’ll need to work on your shoulders and pecs. Bellies are generally safe unless I’m doing abdominal work of course. If it is not possible to put in a different area, maybe try to apply AFTER your appointment instead of before. If that is not possible, we therapists, understand! But we, as therapists will need to work around that area. And if you choose to deliberately withhold that information from us just so you can get an awesome massage, please know OUR health is quite literally at stake in that decision.

cancerFamilies are in the trenches fighting cancer every day. If cancer hasn’t crossed your path,
whether directly or through a loved one, consider yourself lucky. It’s easy to feel helpless when friends and people close to you are struggling. Since September is Childhood Cancer Awareness Month, I’m going to give you some simple things you can do to help those who are facing the dreaded “C” word.

Do. Don’t ask.

It’s easy to say, “let me know if you need anything.” What the family needs is someone to say, “hey, I’m making you dinner.” Or “I’m babysitting – just tell me what day (or night).” Don’t give them the burden of asking. Just offer it.

Gift cards go a long way.

Treatments can sometimes take a patient and family far from home. Do a little research and purchase gift cards for restaurants, hotel chains and gas stations near the hospital they are using. A gift subscription to Amazon Prime or Netflix may be helpful to someone who is bedridden. Some downtime and low key entertainment might be just what they need. And, of course, a massage gift certificate is a perfect way to care for the care givers or the individual.

Remember the siblings.

Cancer affects the entire family. Siblings often are trying to navigate many emotions like fear and jealousy. Commit to a sibling and offer to be there for them. Be a friend. Be someone they can lean on and talk to. If you send the cancer patient a gift, make sure you give them someone of equal to the sibling and include them.

Show up

Shortly after diagnosis, a family will receive calls, cards, and meals. Anything to help soothe the heartbreaking news. After time, these things fade away and the family is still traveling the long road. It could be months or even years. It’s a financial and emotional weight. Keep them in mind and reach out well after the dust settles.

It’s hard to know what to say to a family that’s struggling with cancer. But don’t disappear. Meet them where they are. Sometimes there are no words. Just be present.  Reach out and enlist the help of others to send cards, letters and other nice/silly/fun things that will brighten the family’s day. Even an “I’ve been thinking of you” text speaks volumes.

These tips might not seem like much, but it can mean the world to a family facing cancer. Long days and sleepless nights can be an endless cycle. Little gestures of kindness can bring the family loads comfort.

 

October is Breast Cancer Awareness Month and while there may be some controversy around wearing pink, we are making this month about YOU, whether you have breast cancer or not. (Men, I’m looking at you too.)

Cancer isn’t predictable and it isn’t pretty, but here are a few simple ways to take care of yourself and honor your breasts.

Lead by Example

Perform monthly self breast exams, get knowledgeable about your body, practice self breast-massage, (Don’t know how, ASK ME! and I can guide you!) and schedule regular mammograms. Make sure you schedule a clinical breast exam with your primary care physician, too. Set a good example for the loved ones around you by incorporating healthy practices. This can include, starting a garden, eating as organic as possible, exercising regularly, and packing healthy lunches for your kids.

Talk to Your Family and Friends

Teach your children about self care and good breast health. Tell them about the health risks that come with smoking and alcohol. Talk with your kids, siblings, parents and grandparents about your family’s health history. Consider talking to your sister, mom, or best friend about getting a mammogram – or go together!

Give a Meaningful Gift

Go out of your way to show someone you care. If someone close to you is struggling with cancer, offer to get their groceries, bake them a casserole, take them to a doctor’s appointment, or just be there for them when they need it. A gift doesn’t have to involve money or the purchase of a material item. A sentimental trinket is nice, but having compassion and showing care for someone going through something really difficult is worth so much more.

Get Involved

Consider raising money for a good cause, attending a local event, or starting your own fundraiser during the month of October. Plan a breast health workshop or class in your school, organization, or community. Get involved and make a difference.

Take Care of Yourself

Put in the effort to take care of yourself and this goes beyond eating right. Of course, it’s good to avoid processed foods, but also make sure you are exercising and curbing any habits that negatively affect your health. Replace them with good ones. Turn your 10 minute smoke break into a 10 minute walk break. Avoid staying up to late and try hitting the sack even 20 minutes earlier. Change your monthly Girl’s Night into a Girl’s Spa Day and get massages.

Breast-Massage
It’s all about little changes to make small steps toward health. Breast health goes beyond routine breast exams, it starts with the food you eat and getting outdoors to everything in between.

Whether you are a mother, have a mother, or know a mother, Mothers Day is time to honor those who’ve played this important role. Stepmothers, birthmothers, and godmothers too. While there are as many ways of being a mother as there are moms in the world, one thing is certain: it’s a tough job, but a rewarding one. Here are just three ways massage might be of service to the mothers in your life.

Expecting Mothers

Prenatal massage is a growing field, as training programs for massage therapists increase and old myths about massage during pregnancy fade. Massage can ease the aches and pains associated with increased weight, loosened ligaments, and shifts in center of gravity. There are all kinds of cushions and props that can help pregnant women relax comfortably on a massage table, no matter what their dimensions. Contrary to the old tale, massaging the feet, ankles, back, or even stomach will not cause miscarriage when done appropriately.

Of course, massage therapy, while helpful, isn’t a cure-all. Massage can’t stop that nosy neighbor from touching your belly every time she stops by, or cravings for hot Cheetos and root beer at 2:00 AM. But it can alleviate some of the symptoms that naturally come with growing a new human being, leaving expectant mothers more free to focus on the hope and joy that make pregnancy such a fabulous stage of life.

Adoptive and Foster Mothers

Biological mothers typically get a nine-month period to prepare for parenthood. Foster and adoptive mothers might wait and wait for news … then find themselves parents of a tot or teen virtually overnight. Plenty of mothers juggle soccer practice with homework, but fewer have to add court dates and social worker visits to the mix. Not only can these realities place unusual stress on foster and adoptive moms, their uncommon nature means that it can also be harder to find much-needed understanding and support.

Massage can help manage this stress, for all members of the family. Symptoms of stress, such as headaches, high blood pressure, and anxiety, can all be alleviated by massage. And for children who come from backgrounds where touch was lacking (or inappropriate), pediatric massage can help them develop a healthy relationship with touch. When foster and adoptive mothers learn some of these techniques for themselves, it can also be a way for children and their new parents to bond.

Mothers of Young Children

Unlike pregnant mothers, who often feel like their motherhood draws too much attention to themselves, society tells mothers of young children that their role is all about their children first, second, and last. Getting up in the middle of the night to change a leaking diaper? Chasing a toddler into the street? Taking time off work to care for a kindergartener with an ear infection? No matter, it’s not about you, mom. It’s about your kids.

It’s true, to a point. Who wouldn’t make sacrifices for their children? But one factor that can contribute to a healthy, happy childhood is a healthy, happy mother, and every person deserves to take steps towards their own well being, mothers too!

Massage therapy can help the body rest and recuperate from the effects of a busy schedule, interrupted sleep, and the physical strain of lifting and interacting with growing children. It also provides precious time for moms to focus on their own needs, apart from the responsibilities of mothering. And for those mothers who find themselves dealing with the very real condition of postpartum depression, massage may also play a role (although far from the only one) in an appropriate treatment plan.

Moms who are not moms

For women who don’t have children, by choice or circumstance, Mothers Day can be melancholy. There is great joy in being a Favorite Aunt, Baby Spoiler, and Super Supporter of all her friends with children, but we don’t have specific days to celebrate those roles. Women without children don’t always get all that snuggle time with little ones that raises oxytocin, dopamine, and endorphins. Massage is a great way to get the happy chemicals pumping.  If there’s a woman in your life who loves your children, or makes your job as a parent better or easier, Mothers Day is a great time to recognize her.

There’s no one way to be a mother

Despite what the sensationalist headlines about the Mommy Wars would have us believe, and there’s no one way to take care of your health and wellness. Whether the mother you’re thinking of needs a sports massage after her next marathon or a deeply relaxing hour so she can doze, it’s all worthwhile. So this Mothers Day, let’s do all our moms a favor and call a ceasefire, honoring them in whatever ways make sense for their own lives.

  1. Women are more prone to certain health conditions than men. Women are more likely to experience depression, stroke and rheumatoid arthritis. Some conditions, like osteoporosis, are directly related to women’s hormone fluctuations and smaller frames. Others, like fibromyalgia, are much more common in women, but scientists have yet to figure out why.
  1. Women do not always experience the same symptoms as men with the same conditions. While men are more likely to experience a heart attack than women, women are more likely to die of the same heart attack. The reasons may be related to the publicized symptoms: while everyone knows about chest pressure and pain down the left arm, these are symptoms typically experienced by men. Women may experience dizziness, lightheadedness, or fatigue. Knowing the different ways conditions manifest in men and women can truly be a lifesaver.
  1. Women have different risk factors than men. Remember that bit about women being more susceptible to stroke? In addition to the risk factors shared with men, there are also many women-specific risks, including being pregnant, taking hormonal birth control pills, using hormone replacement therapy, and experiencing frequent migraines. Unfortunately, these additional risk factors don’t always show up in educational materials.
  1. Women and men sometimes react differently to drugs and other treatments. Women wake up faster from anesthesia. Some drugs, like ibuprofen, seem to be more effective in men than women, while others like erythromycin (an antibiotic) work better in women. And of course there are medications typically prescribed for sex-specific issues that can interfere with each other. As an example, acetaminophen (AKA Tylenol) can interfere with the effectiveness of birth control. 
  2.  In spite of all this, women’s and men’s bodies are more similar than they are different. We share 99% of our genetic material with every other person on the planet. We have the same basic structure, suffer from most of the same illnesses, and heal in the same way. A healthy diet, active lifestyle, adequate sleep, and positive attitude are beneficial to men and women alike. There are no studies showing whether massage therapy is better for any one subset of people than others. Maybe that research will be done in the future.
